The undersigned participant (each, a "Participant") in the DARE 2 DREAM INC SOCCER PROGRAM if such Participant is under 18 years of age, then also the parent or legal guardian of such individual (together with the Participant, the "Undersigned"), sign this release and waiver of liability in consideration of participating in all aspects of the Soccer Program. The Undersigned hereby releases, discharges, and indemnifies DARE 2 DREAM INC, and their respective assignees, successors, officers, directors, agents, representatives, employees, sub-contractors, sponsors, partners, members and affiliates (collectively, the "Released Persons") from all present and future liabilities, debts, obligations, costs, expenses, damages, losses, charges, judgements, executions, liens, claims, demands, actions or causes of action of whatever nature or description, in equity or at law, which the Undersigned or his/her child or ward in the Event, expressly including any claims due to negligence of any Released Persons. The Undersigned further grants the Released Persons the right, but does not otherwise impose the obligation, to photograph, videotape and/or otherwise use the Participant's name, face, likeness, voice, and appearance in connection with exhibitions, publicity, advertising, and promotional materials without reservation of limitation. The Undersigned understands, acknowledges, and accepts that this Release and Waiver of Liability is intended to be as broad and inclusive as permitted by the laws of the State of New York and agrees that if any portion of this Release and Waiver of Liability is invalid, the remainder will continue in full legal force and effect. The Undersigned further agrees that any legal proceedings related to this Release and Waiver Liability shall take place in New York City. The Undersigned further understands, acknowledges, and accepts that participation in the Event involves certain inherent risks, including, but not limited to, property damage and serious bodily injury (including death), and agrees that the Participant is voluntarily participating in the Event, and the Undersigned has full knowledge of the risks involved and accepts all risks of participation. The undersigned declares that the Participant is physically fit and has the requisite skill level to participate in the Event. The Undersigned authorizes DARE 2 DREAM INC and/or a party designated by DARE 2 DREAM INC to provide medical treatment to the Participant, at the Undersigned's cost, should the need arise. Furthermore, the Undersigned understands, acknowledges, and accepts that he or she must provide their own medical insurance for the Participant.

By submitting payment for participation in the DARE 2 DREAM soccer program, I acknowledge and agree that all fees paid are non-refundable. This policy applies regardless of player withdrawal, injury, schedule changes, or any other circumstance, whether voluntary or involuntary. Once payment is received, no portion will be returned, credited, or transferred to another season or participant.

As Parent and/or Guardian of the named athlete, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor's life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination, and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to DARE 2 DREAM INC. and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment before the child's admission to the medical facility. Release authorized on the dates and/or duration of the registered season. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence with all cost being my responsibility.
